Inside the bloodstream, NK cells are constantly interacting with other cells. The decision of whether or not an NK cell kills these cells is determined by a balance of signals from activating receptors and inhibitory receptors on the surface of the cell. Activating receptors recognize molecules that are expressed on the surface of cancer cells and infected cells, leading to the activation of the NK cell. Inhibitory receptors act as a control mechanism for NK cell killing.
MHC-I receptors are expressed by normal, healthy cells as a marker that these cells are “self.” Inhibitory receptors on the surface of the NK cell recognize cognate MHC I, preventing the NK cell from activating and killing. Cancer cells and infected cells often lose their MHC I, making them vulnerable to killing by NK cells. Once the decision to kill is made, the NK cell releases cytotoxic granules containing perforin and granzymes, resulting in the lysis of the target cell.
Patients who lack NK cells and cytotoxicity are highly susceptible to viruses, pathogens, senility, and cancer.
